I understand that submittig this form does NOT GUARANTEE that my event will be automatically posted or that my need for massage therapists will be automatically fulfilled. I also understand that National Holistic Institute's current students or recent graduates will contact me themselves if they are interested and/or available in participating in my special event.

I understand and acknowledge that I am legally agreeing to the statements of the language below.
I assume ALL OF THE RISKS, if any, of anyone receiving a massage from employees, volunteers, students or any other person associated with the National Holistic Institute Inc., (collectively “Related Parties”), (“NHI”), that all recipients will advise their massage therapist of any physical conditions they have or may have that could be affected by having a massage, that they are responsible for the risk, if any, of receiving a massage, and that they are waiving and releasing their legal rights to sue for any injury or damages arising out of or resulting from my participation. I further understand that any injury or damages incurred may be the result of negligence, omission or carelessness by the Released Parties. On behalf of myself, my executors, administrators, heirs, next of kin, successors and assigns, and anyone else who might sue on my behalf, I HEREBY WAIVE, RELEASE, AND FOREVER DISCHARGE NHI and its employees, volunteers, and all other persons or entities involved from any and all claims, causes of action, damages, losses (economic and non-economic) and liabilities of every kind (collectively “Claims”) for personal injury or property damage, which may arise out of, result from or relate to participation in receiving a massage.
