Requesting Documents: Step 1 of 2

STEP 1 of 2: Please use the web form below to request your transcripts, duplicate diplomas, or other enrollment verification.

Fee: $15.00 per document

Turn around time: Please allow 2-3 weeks for processing

Method of Payment: Step 2 of 2 will take you to a secure Pay Pal page to complete the request via a major credit card. You DO NOT need a Pay Pal account and will NOT need to create one to pay for your document(s).

Note: If you are a Touch Therapy Institute Alumnus, call Denise Boyle at (818) 788-0824 to obtain transcripts.

For example, if you need a transcript for the NCBTMB Exam, say “NCBTMB exam”. For a duplicate diploma, say “duplicate diploma”. For licensure in a state other than California, tell us the state.
